Where is the practice located?
116 Street Lane, Roundhay, Leeds, LS8 2AL. You will receive our full address on your confirmation email and you can find us on Google Maps.

Yes, this would be very helpful, a health questionnaire will be sent to you after you have made your initial appointment.
May I bring another person into the treatment room with me?
